Even though I didn't have any problems before, I decided to update. Now if I choose a new game+, after I pick any difficulty, an error screen pops up and exits out of the game. But it allows me to continue that save with no problems. I have a physical version of the game. Is there a way to reinstall the update to see if that corrects the problem? Or is there something else I can try?

Depending on what system you have, you should be able to delete the update files and redownload them. Without knowing what system you have, though, I wouldn't know how to go about doing it, and I'd likely end up looking on Google, either way.

Very late reply, but this was a bug introduced with version 1.20 that supposedly got fixed in version 1.21 (but I've still seen some traces off it).
The work-around is to un-equip all gear and shards, save, then start ng+.
The bug for some reason affects equipped items when removing them for ng+. This made it impossible for Zangetsu mode to ng+ because of his sword. If you're on pc you could add a mod to bypass this by just allowing every single item into ng+ (which has its own problems).
